Professor Wai Lun Law, MS

Laparoscopic Colorectal Surgery: The 20 Years On

Professor Law graduated from Faculty of Medicine, The University of Hong Kong in 1987. He obtained the fellowship of the Royal College of Surgeons of Edinburgh in 1991. He was then trained in colorectal surgery and received his overseas training in Houston, Texas. He was conferred the degree of Master of Surgery in 2001 and the research was on surgical treatment for rectal cancer. Professor Law was appointed as the Clinical Professor of Surgery at The University of Hong Kong in 2008. Currently he is the Chief of Division of Colorectal Surgery at Queen Mary Hospital. He is also the Director of Skills Development Centre in the Department of Surgery, The University of Hong Kong.

Professor Law’s main clinical and research interests are in colorectal and minimally invasive surgery. He has been active in developing advanced laparoscopic techniques, transanal endoscopic surgery, robotic surgery and natural orifice transluminal endoscopic surgery. Professor Law is a member of numerous professional societies. He is currently the Vice President of the International Society for Digestive Surgery. He is also on the Membership Committee of the American Society of Colon and Rectal Surgeons.

Professor Law has authored over 110 articles in peer-reviewed journals and has given numerous lectures, both in Hong Kong and overseas. He is currently the editor of Surgical Practice, the Journal of the College of Surgeons of Hong Kong, and the associate editor of the Asian Journal of Surgery. He is also a regular reviewer of numerous scientific journals including Annals of Surgery, British Journal of Surgery, Diseases of the Colon and Rectum, Surgical Endoscopy and Annals of Surgical Oncology.

Professor Enlan Xia, MD

Twenty Years History of Gynecological Endoscopic Surgery in China

Dr. Enlan Xia is Professor and Director of the Hysteroscopic Center at Fuxing Hospital, which is affiliated with Capital Medical University. She received her medical degree from of Northwestern University in China in September 1955.

She is the founder and a pioneer of Hysteroscopy in China. Since May 1990 she has worked in Gynecological Endoscopy especially in Hysteroscopy. She established the Beijing International Hysteroscopic Center. The center’s goals are research the fundamental theory and technique of Hysteroscopy, to treat patients with gynecological endoscopy, and to spread the technology for other gynecologists. To date, she has performed more 30 thousand cases of hysteroscopic surgeries and 7000 laparoscopic procedures. Dr. Xia has presided over the Beijing International Symposium of Gynecological Endoscopy 19 times and over the Hands-On Workshop of Gynecological Endoscopy in Beijing 37 times.

Dr. Xia has published 207 papers, and as a chief editor published in Gynecological Endoscopy and Hysteroscopy and Atlas. She has also translated and published Atlas of Colposcopy, Complications of gynecological endoscopy, and Hysteroscopy-Visual perspectives of uterine anatomy, physiology and pathology. She has also completed Scientific and technical research on the Clinical Application and Fundamental Research on Hysteroscopy.

She is the recipient of the National second award for advanced science and technique in China in 2004.

Smart Surgeons Learn From Their Mistakes, Brilliant Surgeons Learn From Other Surgeons' Mistakes: Prevention and Management of Laparoscopic and Endoscopic Surgical Complications

Half-Day (8:00am-12:00pm; 4 AMA PRA Category 1 Credit(s)™)

All abdominal and pelvic MIS procedures carry an inherent risk of complications. This interactive course will present a philosophy for the prevention and management of complications during minimally invasive surgery of the abdomen and pelvis. Video segments and case presentations will be used to demonstrate principles and stimulate discussion based on participant needs identified by responses to the pre-course questionnaire. Source material will be obtained from students’ “real world” experiences and supplemented with materials selected by the faculty panel and customized based on pre-course questionnaire results. An interactive student and faculty discussion format will be utilized. This course will discuss management paradigms to prevent, recognize, and treat complications appropriately. Careful, methodical assessment and strategies for appropriate action will be stressed. Topical presentations based on student input will highlight detailed surgical anatomy, sound surgical principles and careful technique. Participants are strongly encouraged to submit video or other case material (anonymously) to maximize topical relevance for their individual practice and needs.

OBJECTIVES
Course participants will be better able to:  • Recognize and decrease the risk of complications in laparoendoscopic surgeries;  • Identify and discuss specific conditions affecting appropriate patient selection;  • Discuss indications, contraindications and limitations of MIS procedures and technologies;  • Understand relevant surgical anatomy and potential technical pitfalls;  • Develop management paradigms to prevent and treat complications;  • Describe the rationale and timing of conversion to open procedures.

Reduced Port MIS, More Than an Incision Decision

Full-Day (8:00am-4:30pm; 7 AMA PRA Category 1 Credit(s)™)

Leading practitioners in the fields of general, gynecologic, and urological MIS will discuss their experiences with single port access (SPA), or single incision laparoscopic surgery (SILS), and natural orifice transluminal enteric surgery (NOTES). The advantages, risks, and disadvantages of SPA and NOTES vs. traditional MIS will be discussed and debated, as well as future applications of the respective technologies. Current positions of NOSCAR, the combined ASGE and SAGES working group on the development, research, and introduction of NOTES technology will be discussed.

OBJECTIVES
At the conclusion of this course, participants will understand the indications and contraindications of SPA, be familiar with facilitating instrumentation and technology, and understand the choreography and technical concepts of SPA surgery. They will be familiar with the evolving technology of NOTES, understand the potential risks and benefits peculiar to NOTES and SPA, and be able to compare these two technologies and judge whether either or both of these technologies would be applicable to their individual surgical practices. They will increase their understanding of NOTES and the positions and recommendations of NOSCAR.

This Master Class is for advance laparoscopic surgeons specializing in the treatment of endometriosis and fibroids. Leading practitioners in the fields of gynecology will discuss their experiences in the evaluation and management of these pathologies. Given the potential for involvement of nerves, blood vessels, intestinal tract, or urinary tract, and the various faces and behaviors of fibroids, surgery for endometriosis and fibroids can be very difficult. Minimally invasive surgical and non-surgical approaches will be presented.

Objectives:

At the conclusion of this course, participants will be able to 1) Understand the various pathologies related to endometriosis and fibroids 2) Learn what to anticipate during surgery and whether to refer the patient. 3) Learn a standard plan of attack for each anatomic problem 4) Assemble a multi-disciplinary surgical team 5) Analyze the role of different surgical and non-surgical minimally invasive techniques for the treatment of uterine fibroids and endometriosis.

The general surgery postgraduate course has been expanded for 2011 and will emphasize “tips, tricks, and masters”. The nationally and internationally recognized faculty members have been carefully selected based on their demonstrated expertise, in depth clinical experience and their passion as educators. The faculty will share their knowledge, experience, and best practices while emphasizing the “tips and tricks” to facilitate efficient, safe and reproducible surgery that is tailored to real-world clinical problems and scenarios. The format will be interactive and based upon the needs of the audience using real-world clinical examples. Accomplished surgeons in practice and senior trainees interested in MIS are encouraged to attend and participate in this course.

Course Objectives:

The following topical areas will be discussed: single incision & reduced port surgery; improving ergonomics and reducing injuries; GERD/hiatal hernia management, solid organ surgery, bariatric surgery issues for general surgeons, selecting hernia prosthetics, and surgical skills improvement.

Course attendees will increase their knowledge and understanding relative to patient selection, positioning, equipment selection, technical details and pearls, managing and preventing intra operative complications. Emphasis is on “tips and tricks” that facilitate conduct of these procedures and improved outcomes. Attendees will be better able to assess their own clinical performance relative to established benchmarks and will learn additional strategies to solve clinical problems.

ORReady is a worldwide, multi-specialty initiative meant to encourage steps that are known to improve surgical outcomes and save lives. If the current guidelines such as Check Lists, Time Outs and Warm Ups are followed routinely, it is estimated that six million patients around the world could have better outcomes through error reduction. That's 2-3% of the approximately 250 Million surgeries that are performed worldwide each year. This course is meant to improve awareness of safety and outcome issues and focus on how to implement systems that promote the penetration of these measures into the culture of the operating room environment and beyond. Validated warm-up procedures, introduction of empowering patients and referring physicians in the peri-operative process; and suggestions for better communication amongst the peri-operative team will be presented. Nurses, surgical technicians, surgical physician assistants, surgeons and administrators are all invited.

Objectives:

1) Describe the etiology of Operating Room Errors, Where does the error begin and time to discovery

2) What is the impact to the hospital, physician and patient of an error in the Operating Room?

3) Define the “Team Approach” to error management and prevention from patient admission to discharge.

The implications of these findings for the design and conduct of surgical education are profound.

In this workshop, Hilliard Jason, MD, EdD,and Jane Westberg, PhD, former Co-Directors of Education, SLS and Clinical Professors, Family Medicine at the University of Colorado Denver, will share with you some of the highlights of these emerging understandings, and will encourage you to reflect on whether any changes may be needed in the ways that you and your colleagues now teach and in the ways you pursue your own ongoing learning.

This workshop will provide you with opportunities to:

1. Review and reflect on some of the findings of recent education and brain science that are relevant to surgical teaching and learning.

2. Think through the extent to which you and your colleagues are providing educational experiences for your students, residents, and others in ways that are consistent with the implications of good science.

DIRECTED ENERGY FOR TISSUE ABLATIVE THERAPIES

The use of extreme temperature for tissue ablation is constantly developing. Different energy sources such as Radiofrequency (RFA), Laser, Microwave, Focused ultrasound are known to be efficient ablative modalities. These modalities enable minimally or even non invasive approach to pathological tissues, yet they require accurate guidance, control and verification of the procedure. We shall review recent advances in heat ablation technologies:

Percutaneous RFA for the ablation of kidney tumors in select patients with small renal masses. This is based on published series demonstrating comparable disease-specific survival rates to surgery with less morbidity. Experience of 300 cases, including Laparoscopic ablation will be presented.
Plasma energy for ablation of Malignant and Benign gynecological pathologies.
Stereotactically guided Interstitial Laser Therapy (ILT) of small breast cancers was first reported over a decade ago. The methodology and the outcome of two patients who were treated with ILT will be presented.
MR guidance for focused Ultrasound heat ablation is a totally non-invasive modality with an optimal real-time guidance and control of the procedure. Recent advances of this modality will be reviewed.

Guided in-situ ablation of tissue pathologies by either generation of heat or cryo, is a promising trend that requires further research in-order to become accurate enough and an inevitable adjunct to minimally invasive surgery.

SMALLER OR ALTERNATIVE ACCESS SURGERY: SHOULD I DO IT? IS IT BETTER FOR MY PATIENT?

Over the past four years, we have seen a re-emergence of the desire to further minimize the trauma and pain for surgical patients. The sudden growth of Single Port surgery and the ongoing presence and research of NOTES has demonstrated our clear commitment to push the envelope of minimizing surgical trauma, but also reflects our desire to continually improve what we do.

Now that we know what we can do, it is certainly time to bring all this together and ensure that we move safely in the right direction. Proof of concept should not only show us what we can do, but should lay the platform to expand what we can do at the next level.

Do we decrease the number of incisions, decrease the size of incisions, and move the incisions to conceal them better? Is the umbilicus the best place to approach all this? Isn’t the umbilicus always a “free” port site?

A series of brief presentations by experts in the field will help the attendee to develop a critical eye and assist them in the adoption of limited/alternative access surgery into their practice, both in a stepwise progression and as a new platform. The attendee will be equipped to identify the safe parameters, exclusion criteria, strategies for adoption, and outcome data by the end of this session.

This session will discuss the role of organ-sparing surgery in minimally invasive surgery from the perspective that organs with no intrinsic pathology should not be removed if possible. Established in 1991, the Excel Award has been presented to 26 surgeons who have made outstanding contributions to laparoscopy, endoscopy, and minimally invasive surgery. The Excel Award may be presented to an individual, in any field, who has made outstanding contributions to laparoscopy, endoscopy, and/or the field of minimally invasive surgery. Previous recipients have been from various specialties and various nationalities.

Dr. Michel Gagner was born in 1960 in Montreal, Quebec, Canada. Dr. Gagner obtained a Diploma in Sciences at the Seminaire de Sherbrooke in Sherbrooke in 1978 and his M.D. from the Faculte de Medecine de l’Universite de Sherbrooke in Canada in 1982. He did his surgical training at McGill University in Montreal from 1982-1988. During his residency he completed 2 years of a Ph.D. program on human lipolysis in sepsis at the Royal Victoria Hospital, McGill University from 1984-1986. Dr. Gagner completed also fellowships in Hepatic surgery at Hopital Villejuif in Paris, France and Pancreatico-Biliary surgery at Lahey Clinic Medical Center in Burlington, Massachusetts, from 1989-1990.

Upon completion of his fellowships, he was recruited to the Universite de Montreal School of Medicine, Hotel-Dieu de Montreal, where he was Assistant Professor of Surgery (1990-1995). He then spent several years at the Cleveland Clinic Foundation in Cleveland where he co-founded the Minimally Invasive Surgery Center (1995-1998). He was appointed the Franz Sichel Professor of Surgery and director of the Minimally invasive Surgery Center of Mount Sinai School of Medicine in New York, from 1998 to 2003. He joined Weill Medical College of Cornell University (New York) as Professor of Surgery and Chief of the section of laparoscopic and Bariatric surgery (2003-2007). He was until recently, chair of dept. of Surgery at the Mount Sinai Medical Center (Miami) and Professor of Surgery at Florida International University, currently Professor of Surgery in Montreal.

Dr Gagner is known for his contributions in the field of Minimally Invasive Surgery, in particular the first description of laparoscopic adrenalectomy for Cushing syndrome and pheochromocytoma (1992), first description of laparoscopic pancreatectomy (distal and proximal) (1992-93), first description of endoscopic neck surgery with parathyroidectomy in 1995, first transgastric cholecystectomy in 1997 (NOTES), first description of laparoscopic duodenal switch for obesity in 1999 and laparoscopic sleeve gastrectomy in 2000. More recently, he was co-surgeon for the first World transatlantic robotic cholecystectomy, between New York City and Strasbourg (France), the only surgical article published in Nature in 2001.

He has over 250 published journal articles, 40 book chapters and 6 books on Minimally Invasive Surgery. He has been visiting professor and operated in over 60 institutions in 48 countries. He has held prominent positions in more than 35 societies and organizations. He has been on the editorial boards of 12 surgical journals.

Dr Gagner also has received a number of honorary memberships and awards throughout his career. His most recent contributions focus on innovative upper digestive tract surgery like bariatric and endoluminal gastric procedures.

Since the rise of science fiction writing by the Fathers of Science fiction (HG Wells and Jules Verne) at the turn of the 20th Century, and the "Big 3" of Modern Science Fiction (Isaac Asimov, Arthur C. Clarke, Robert Heinlein) during mid century, there has been a fascination with the fertile minds of these pioneers, as well as whether their predictions would actually come true. An amazing number of 'classic' writings are now considered 'old fashion' - Verne's 20,000 Leagues Under the Sea (submarine), Wells' First Men in the Moon (space travel), Asimov's I, Robot and the"3 Laws of Robotics" (robots), Clarke's 2001: A Space Odyssey (GPS satellites), yet these 'crazy science fiction fantasies' are now critical infrastructure for the world as we know it today. This panel will include prominent science fiction writers who will give their views on what they think would (or would like to see) become possible in the future of medicine.

CANCELLATION POLICY

Full registration fees are refundable if registrant cancels before August 17, 2011. An administrative fee of $150.00 will be deducted from fees for cancellations postmarked on or after August 17, 2011 through August 31, 2011. Refund requests will not be considered after this date, including visa denial refunds. All requests for refunds must be made in writing and received by SLS, attention Lauren Frede, by the appropriate dates. Refunds will be processed within 6 to 8 weeks after the conference.
